Note: All documents are to be submitted together. Incomplete packets will be NOT be accepted.  

 

Year 1 of Catechisim & Continuing Fatih Formation:   Items marked with an asterisk (*) are due at time of registration.

Parents:

This is an exciting time for your child preparing for the Sacrament of Reconciliation. (Depending on where we are with COVID-19, we hope and pray to be back in the classroom. Each Classroom will only have (TBD) children with social distancing and following all protocols for a safe environment) 

  1. Completed Registration Packet*
  2. Baptism Certificate (if not Baptism please bring birth certificate of child)*
  3. $80 Tuition* 
  4. $10 DDF if register in our parish or $20 DDF for non-parishioners of our parish
  5. You will receive a text book
  6. Step by step Instructions

 

Second Year Fisrt Communion & Children RCIA

Parents:

This is an exciting time for your child preparing for the Sacrament of Reconciliation. (Depending on where we are with COVID-19, we hope and pray to be back in the classroom. Each Classroom will only have (TBD) children with social distancing and following all protocols for a safe environment) 

  1. Completed Registration Packet*
  2. Baptism Certificate (if we have not received it)*
  3. Birth Certificate (If the child is not baptized)* 
  4. Year 1 Tuition  paid in full*
  5. Parent Meeting and Holy Hour Card *
  6. $10 DDF if register in our parish or $20 DDF for non-parishioners of our parish

Sponsor:( Sponsor can not be a parent)

  1. Sponsor Form, signed and sealed by Sponsor's home parish
