 Note: All documents are to be submitted together with the full tuition. Incomplete packets will NOT be accepted.

 

First Year Confirmation Formation:  Items marked with an asterisk (*) are due at time of registration.

Teens

• Confirmation Retreat
• 8 Nights of XLT Adoration (1^st Thursday of every month beginning in October 2024)
• SOUL Nights
• 3 Service Projects with written reflection
• Participate in Catechetical Experiences 
• Saint reflection project 

Parents:

• Copy of Baptism and First Communion Certificate of teen
• Completed registration packet.
• Tuition Due at Time of Registration: $230 per teen, plus $20 DDF for non-Registered parishioners
• Participate in your child’s formation by attending the following:
• 7 Parent Formation Sessions (every 2nd Tuesday of every month beginning in October)
• 8 Nights of XLT Adoration (1st Thursday of every month beginning in October)
• Parent Orientation: September 17, 2024, at 6:00pm
• Parent Retreat
  

Second Year Confirmation Formation:

Teens 

• Confirmation Weekend Retreat
• Attend Sponsor/Candidate Day
• 8 Nights of XLT Adoration (1^st Thursday of every month beginning in October 2023)
• 8 SOUL Nights – 1^st Saturday of every month beginning in October
• Service Project with written reflection
• Participate in Catechetical Experiences beginning
• Saint reflection project 

Parents:

• Copy of Baptism and First Communion Certificate of teen on file
• Completed registration packet
• Tuition Due at Time of Registration: $250 per teen, plus $20 DDF for non-Registered or active parishioners
• Participate in your child’s formation by attending the following:
• 7 Parent Formation Sessions (2nd^rd Tuesday of every month beginning in October)
• 8 Nights of XLT Adoration (1^st Thursday of every month beginning in October)
• Parent Orientation: September 10, 2024, at 6:00pm
• Parent Retreat

Sponsor:( Sponsor can not be a parent)

• Sponsor Form, signed and sealed by Sponsor's home parish*
• ATTEND 2 days Sponsor/Candidate Day

Confirmation Sponsor Requirements:

The individual must be at least 18 years of age (per Diocesan policy).

He or she must have received the sacraments of Baptism and Confirmation in the Catholic Church.

If he or she is married, the marriage should have taken place in the Catholic Church.

The person must have the intention, the ability, and adequate time to be a Sponsor.

The Sponsor should live according to the Catholic Faith: attend Mass every week and receive the sacraments of the Church regularly, especially Reconciliation and the Eucharist.

The Sponsor should practice the Faith within a Catholic Parish community and live a moral life.

He or she must be able to attend a Sponsor/ Candidate session.

Must turn in a completed Sponsor Form with the parish seal and Pastor’s signature from the parish in which they attend.
